  2. Has gambling ever made your home life miserable?
  3. Does gambling affect your reputation?
  4. Have you ever felt remorse after gambling?
  5. Have you ever gambled to get money to pay your debts or to
    solve your financial difficulties?
  6. Does gambling diminish your ambition or effectiveness?
  7. After losing, do you feel you have to come back as soon as possible and make up for your
    losses?
  8. After winning, do you have a strong desire to come back and win more?
  9. Did you gamble a lot until you lost your last penny?
  10. Have you ever borrowed anything to finance your gambling?
  11. Have you ever sold anything to finance a gamble?
  12. Were you reluctant to use "gambling money" for normal expenses?
  13. Doing gambling makes you reckless about your own welfare or your
    family?
  14. Have you ever gambled longer than planned?
  15. Have you ever gambled to escape worry and trouble?
  16. Have you ever committed or considered committing an illegal act to
    finance a gamble?
  17. Does gambling make you have trouble sleeping?
  18. Do arguments, disappointments or frustrations create within you an urge to
    gamble?
  19. Have you ever had the impulse to celebrate some good fortune by
    gambling for a few hours?
  20. Have you ever considered self-destruction or suicide as a result of
    of your gambling?
