Pubic symphysis is a painful condition of pregnant women, which, however, despite the feeling of great discomfort, is not dangerous. The main clinical manifestation is symphysis pain, worsening when standing up or lifting weights. Outside of pregnancy, symptoms may not appear until after the baby is born. Perineal discomfort may radiate to the sacrum.

Pubic symphysisis a painful ailment, affecting mainly pregnant women. The increasing concentration of estrogens in the blood and the increase of the hormone relaxin are responsible for the physiological loosening of the pubic synchondrosis and the connections between the sacroiliac joints during pregnancy. All these changes are intended to prepare the pelvis for childbirth. This facilitates the physiological delivery and the passage of the baby through the mother's skeletal system. Symptoms of symphysis divergence appear mainly in the third trimester, when the fetal head is lower and lower and presses against the pelvic east.

Symptoms characteristic of the discussed pathology are:

  • pain in the symphysis area, increasing when walking

Rapid medical intervention is necessary, not only to reduce the symptoms, but also remember that symphysis breakdown is a contraindication to childbirth by means of natural means. Therefore, such pregnancies should be terminated by elective caesarean section before 40 weeks of gestation. The worsening of pain may be an indication for faster termination of pregnancy.

Reasons for the dissolution of the pubic symphysis

Damage to the symphysis in the form of a dilated joint space in obstetricians results from a perinatal trauma. Such an injury results from:

  • long second stage of labor, this is the time from full dilation to birth
  • disproportionatelabor, i.e. too much weight of the child in relation to the relatively narrow bone pelvis
  • forceps delivery

The formation of a symphysis pubis in the injury mechanism is rare, especially in the era of advanced ultrasound techniques. Any suspicion of a birth during physiological delivery is an indication for the inversion of delivery to caesarean section.

Diagnosis and treatment of symphysis in pregnancy

The diagnosis of the described pathology consists in collecting an accurate interview and determining the characteristic ailments. From imaging studies, ultrasound is recommended, where the examiner finds a small gap between the pubic bones. The prognosis is good because we are not talking about a rupture of the pubic symphysis, but a slight separation of the bones that make up the pubic symphysis. Treatment consists of resting and avoiding heavy lifting.

In the field of pharmacology, there are proposed measures to reduce pain, ie non-steroidal anti-inflammatory drugs, used very carefully among pregnant women due to the risk of transmitting active substances to the fetus. You can try rehabilitation, where appropriate exercises should effectively reduce pain. Of course, you must not introduce exercises on your own during pregnancy, but under the strict supervision of a doctor.
