The problem you described is called a short frenulum. This causes the erection problems described by you, pain, and sometimes during intercourse, the frenulum tears and bleeds. You can perform frenulum plastic surgery, which allows you to freely withdraw the foreskin and free from pain during intercourse. Relapses are probably due to incomplete rupture of the frenulum and the problem will continue to occur. I suggest visiting a urologist.
