An application for rehabilitation as part of ZUS disability prevention can be issued by any doctor who carries out treatment and knows the history of the disease. It does not matter if it is a primary care physician or a specialist (even a private one who does not have a signed agreement with the National He alth Fund).

It should contain the following data:

  • name and surname of the patient
  • PESEL
  • home address and telephone number
  • type of disease the patient is struggling with

However, the most important thing is the opinion that there is a chance of regaining ability to work after rehabilitation. The doctor may write such a request on his own or on the form available on the ZUS website.

The waiting time for the decision whether ZUS will issue a referral to a sanatorium is from 3 to 8 weeks.

The application is submitted together with the medical documentation at the nearest ZUS unit. However, before ZUS finally decides about a referral to a sanatorium, it may have the applicant examined by a ZUS certified doctor.

Its task is to assess the chances of returning to work after rehabilitation, taking into account age, occupation and activities at work and working conditions, but above all, how the course of the disease affects the body.

The examination by the medical examiner can be avoided if the medical records are sufficient to support the decision.
