He althcare benefits are available to all citizens of the Republic of Poland. Find out which documents confirm the payment of he alth insurance contributions.

Who pays for he althcare benefits

  • NFZ -in the case of insured persons - persons for whom contributions are paid tohe alth insurance , paying the contribution themselves, persons covered by insurance voluntary, persons undergoing an adaptation internship, Polish language course and preparatory courses to study in Polish, referred to in the provisions on higher education, family members of persons covered by universal insurance.
  • State budget-for non-insured persons
  • Citizen that is you-for driving license certificates and other medical certificates issued at your request, with the exception of certificates needed to obtain a nursing allowance or social assistance benefits; for services included in the list constituting an appendix to the Act onhe althcare servicesfinanced from public funds.

What documents are needed to take advantage of he althcare services

When going to the doctor, take a document confirming payment of contributions with you. At present, the following apply:

  • retirement or disability pension claim,
  • current family he alth insurance card or application for insurance (ZCZA or ZCNA form) with a valid student / school ID card,
  • in the case of students insured by the university - valid student ID with a document confirming application for insurance,
  • confirmation of contribution transfer (for self-employed persons and voluntarily insured persons, retirees and pensioners)

Ultimately, however, it is to be the European He alth Insurance Card.
In the event of an emergency, you can present the document at a later date, but not later than within 30 days from the date of commencement of the provision of he althcare services, if you are still in hospital, or within 7 days from the date of termination of the provision of care he alth.

Where you can usefrom he althcare services

In clinics, offices and hospitals that have signed an agreement with the National He alth Fund. They should bear the logo of the National He alth Fund.
