To obtain it, please contact an orthopedist, neurologist, rheumatologist or medical rehabilitation physician with a request to issue an application for the purchase of the appropriate equipment. The application (on a special form) may be issued by a doctor who signed a contract with the National He alth Fund. An adult is en titled to a new stroller every 5 years. NFZ for a manual wheelchair (code NFZ 9194.01) pays an extra PLN 800. In this amount, you can choose a good stroller, which will make it easier for your husband to move freely (ordinary strollers cost PLN 600-1000). If the doctor decides that, for example, a light active stroller (code NFZ 9194.02) will be better for the husband, he will issue an appropriate order, and the reimbursement will be PLN 1,500.
