Will that change soon? The Polish Dermatological Society once again calls for certain treatments to be performed only by doctors, and that the issues related to aesthetic medicine treatments are legally regulated. What does this mean for customers?

Who should perform aesthetic medicine treatments?

Treatments in the field of dermatology and aesthetic medicine - incl. with the use of botulinum toxin or hyaluronic acid, platelet-rich plasma, cryotherapy, lifting threads or laser therapy are extremely popular in our country. And offered in many beauty salons. Polish law does not clearly define which treatments can be performed by a cosmetologist and which by a doctor.

It's unacceptable - argue dermatologists associated with the Polish Dermatological Society, the Association of Aesthetic Dermatologists and the Polish Society of Aesthetic Medicine and Anti-Aging. According to doctors, these aremedical services, not cosmetic services , during which the skin layers are broken and drugs or medical devices are used. Their use by people without proper knowledge and experience may lead to deplorable consequences, including complications that require long and specialized treatment - skin infections, inflammatory infiltrates, HIV and HCV infection, as well as disfigurement, disability, and even death.

In additionsuch treatments are often performed in places that do not meet sanitary and epidemiological standards(cosmetic or hairdressing salons do not have to follow them). The person who performs them, and is not a doctor, does not bear any criminal or financial responsibility according to the law.

Another huge problem is the fact that in beauty salons, electrocoagulation, cryotherapy or laser therapyoften remove skin lesions that may be cancer- they are removed by people who are not doctors, it may lead to the abandonment or significant delay of cancer treatment.

As prof. dr hab. n. med.Barbara Zegarska, the main organizer of the congress of the Aesthetic Dermatology section of PTD JADE 2022: - It is not about fighting cosmetologists, whom I have been training for years, like other colleagues, I employ in my office and I greatly appreciate their knowledge and competences. Our goal is to care for the he alth and safety of patients. Each medical procedure has complications. The doctor has the knowledge and competence to deal with them, has the ability to apply appropriate treatment of both complications and side effects. In addition, the doctor has legal responsibility towards the patient. - underlines.

For what treatments to the cosmetologist?

Doctors emphasize that the cosmetology study program does not include treatments in the field of aesthetic medicine. As explained by prof. Aleksandra Lesiak, no cosmetologist has the right to perform procedures that violate the cutaneous-epidermal border, and therefore, among others, to perform punctures.

Therefore, dermatologists propose to separate the competences of cosmetologists and medics so that the former can only perform cosmetic, care and beauty treatments that do not interfere with the continuity of the skin and do not require the use of drugs or medical devices, and also closely cooperate with doctors dermatologists in the care of pathologically changed skin.

Which treatments are safer to perform at the doctor's?

Which treatments, currently available in some beauty salons, should absolutely be performed by a dermatologist?

According to doctors, these should be all medical procedures that are considered aesthetic medicine treatments, so:

  • injection treatments - volumetry, lifting, modeling with hyaluronic acid in various degrees of cross-linking
  • treatments with biostimulants - polylactic acid, calcium hydroxyapates, caprolactone, amino acids, collagen, biostimulators based on hyaluronic acid
  • injection procedures related to the introduction of drugs - botulinum toxin, steroids, hyaluronidase
  • treatments with autologous fat
  • substances administered by injection, needle mesotherapy - subcutaneous puncture, intradermal non-drugs used for aesthetic purposes and introduced by injection or puncture (does not apply to tattoo dyes, permanent makeup)
  • lifting threads - all types of the so-called hooked and hookless
  • platelet-rich plasma
  • devices that interact with the energy of electric current and electromagnetic or mechanical waves with power densitiesexceeding the value of 1000mW / cm2 at the point of the greatest impact on the tissue
  • removal of skin lesions using CO2 lasers, electrocoagulation and cryotherapy
  • deep peels
  • dermabrasion
  • ablative lasers.

What would these changes mean for customers?

The separation of the competences of cosmetologists and doctors was one of the most important topics at the recently completed congress of the PTD Section of Aesthetic Dermatology - JADE 2022. For years, dermatologists have been calling for the issues of aesthetic medicine treatments to be legally regulated, their position is also supported by the presidium of the Supreme Medical Council. The doctors recently asked the Minister of He alth for help in regulating this issue.

Perhaps the price of these treatments is sometimes higher in doctor's offices than in non-medical offices, but this is mainly related to the price of the product itself. Each of us doctors supplies the preparations directly from the distributor, which is provided under appropriate conditions and is also admitted to trading. Experts emphasize that the doctor, as a medical graduate, knows anatomy very well, unlike a beautician or tattoo artist, the patient can be sure that even in the case of side effects or complications, he can count on the use of appropriate medical procedures.
