They are like a second skin for people with blistering epidermal detachment (EB) - a rare, genetically determined disease characterized by an extremely delicate and vulnerable epidermis. Even a minor trauma can lead to the detachment of a large area of ​​the epidermis from the dermis. In people with the most severe form of the disease, up to 70 percent of the body's surface is covered with wounds. Every activity, walking, eating it all involves great suffering. The disease cannot be cured, but the patient's suffering can be alleviated by using specialized dressings (the most commonly used dressings are Mepilex).

Dressings (plasters) - properties and indications for use

A specialized dressing is designed to minimize the risk of injury and pain to the patient, both when wearing the dressing and when changing it. The dressing seals the edges of the wound, preventing the exudate from leaking onto the skin surrounding the wound, and allows excess exudate to evaporate while being a barrier to bacteria and viruses.

Dressings not only heal wounds, but also protect the skin against injuries.

Thus, it provides the right conditions for the healing process. The dressing is flexible, so it can be used on wounds that are difficult to dress. Its use enables easy viewing of the wound during therapy and evaluation of the healing process. In addition, the dressing allows the use of local treatment - ointments or creams. It is completely waterproof, so patients can bathe in it or take a shower.

The dressings can be used for traumatic wounds, diabetic wounds, postoperative wounds, leg ulcers (arterial, venous), bedsores, abrasions, burns, wounds with necrotic tissue (in combination with gel) or wounds with affected / sensitive surrounding skin. The dressings used in EB are reimbursed, but the Ministry of He alth has again decided to reduce it, so the prices of patches have drastically increased. In January 2015, patients had to pay PLN 248 for them, a year later - after the first increase - PLN 2,358, and in January 2022 - PLN 3,478. There are other dressings on the reimbursement list, but they contain too strong adhesive that breaks the skin when changing them.

In addition to dressings, which the patient needs several hundred a month (because he changes them several times a day, every day), the treatment also includes bandages, gauze, needles, disinfectants, saline, liters of ointments and dermocosmetics, nutritional supplements, vitamins , dietary supplements - 100% paid Therefore, the total monthly cost of treating a patient with EB is estimated at PLN 5,000 to 7,000. After the increase in the price of patches, the costs increase, which is beyond the scope of the sick.

Mölnlycke He alth Care Polska Sp. z o. o will provide patients with EB 2,000 Mepilex Transfer dressings, 15x20 cm. This is good news for patients, but it is an emergency. Dressings will be handed over in accordance with the procedure and formula proposed by the Ministry of He alth. The manufacturer also proposed long-term solutions that would stabilize the situation in the future and avoid an increase in subsidies to dressings used in the blistering separation of the epidermis.

The current increase in the surcharge for patients, which may reach even several hundred zlotys within a month, results from the mechanisms of the Reimbursement Act. Although the official selling price is maintained, the increase in the surcharge is due to a change in the base of the funding limit, which is not directly influenced by Mölnlycke He alth Care.

The official selling price for Mölnlycke He alth Care products in the indication for patients with EB was agreed with the Ministry of He alth in 2016. In April and August 2016, negotiations were conducted, as a result of which official sales prices for silver dressings were reduced by an average of 6.9%, and for other dressings, prices were reduced by an average of 4.1%. These arrangementsentered into force on 1.11.2016 with the announcement of the new reimbursement list and have not changed since then.

In the current situation, Mölnlycke He alth Care will exceptionally reduce the sales price for the Mepilex EM dressing, 17.5 x 17.5 cm, so that patients do not have to pay extra for it from March. However, it is also a temporary solution, because with the next list of reimbursed drugs, the basis for the funding limit may change again, which may result in an increase in patients' subsidies for dressings. The best and permanent solution for patients would be to create a separate reimbursement group, in which there will only be products dedicated to patients with EB with zero patient surcharge. Patients' associations are trying to create a special group, because they are the basic dressings used in the world in the treatment and care of EB for patients. As agreed with the Ministry of He alth, by January 20, representatives of patient organizations will prepare a draft of a new division of limit groups.
