The formula of the prescription is specified by the legislator. In theory, all doctors know how to fill the prescription correctly, in practice it happens that the patient cannot buy the drug because the prescription is not properly issued. What should a correctly filled prescription look like? What data should be on it?
Prescription templateis specified in Annex 6 to the Regulation of the Minister of He alth, Journal Of Laws of 2007, No. 97, item 646 as amended d.). According to it, the medical prescription has dimensions of not less than 90 mm (width) and 200 mm (length) and not more than 110 mm (width) and 215 mm (length). Below we present a formula of the recipe. We also explain whata correctly filled prescription should look like , what data should be on it so that it can be redeemed in a pharmacy.
1. The number of the Provincial Department of the National He alth Fund is entered here, appropriate for the patient's place of residence for MOW NFZ - 06 or the symbol of the EU country in the case of the provisions on coordination
2. The regulation defines the following powers: IW - military invalids; IB - war invalids; ZK - meritorious honorary blood donors; deserving transplant donors; AZ - patients with asbestosis; PO-soldiers of basic military service, WP- soldiers in active military service; DN - uninsured children under the age of 18; CN - uninsured persons who are en titled to benefits related to pregnancy, childbirth and puerperium; BW - a patient en titled to he althcare services on the basis of a decision of the commune head (mayor, president) of the commune competent for the patient's place of residence.

4. The person authorized to issue prescriptions may prescribe up to three prescriptions for consecutive periods of use not exceeding a total of 90 days of use. In this case, enter the date from which the filling of the prescription can take place here, otherwise enter the character "X".
Prescription formula: rules for filling prescriptions
- Prescriptions should be filled in legibly and indelibly.
- A medical prescription (white) may contain up to five ready-made drugs, foodstuffs for particular nutritional uses, medical devices or one prescription drug.In the case of prescriptions for psychotropic and narcotic drugs, one drug should be placed on one prescription.
- If the patient is under 18 years of age and the person issuing the prescription cannot determine the age of the patient based on the PESEL number, enter his age in the "Patient" field.
- Any changes and corrections to the prescription (with the exceptions contained in the Regulation on medical prescriptions) may be made only by the person authorized to issue prescriptions, a signature and personal stamp are placed in the place of the change.
- Details of the he althcare provider and the prescribing physician must be included on the prescription in the form of a seal, an imprint, or a sticker attached to the prescription in a way that prevents its removal without destroying the prescription print.
- Prescriptions for non-reimbursed or uninsured drugs must include (at least) the patient's details, the name of the prescribing physician, the contents of the prescription and the date of issue. In such cases, the doctor does not need to use coded prescriptions.
