Hello again. Thank you for your answer regarding the need to report the suspected epilepsy to the driver. Therefore, I have another question: how is this report about suspected epilepsy in the driver made - on formalized forms (where to get it, if any)? By letter? By phone? Electronically? Who does it: the doctor who diagnoses the disease, or maybe the head / director of the He alth Care Center?

A doctor who, during the practice of the profession, found that an applicant for a driving license or who holds a driving license has had an epileptic seizure or a suspected or existing epilepsy, regardless of the circumstances, should immediately notify the driving license authority of the necessity to to assess the he alth predisposition of this person to drive. If the medical certificate states that there are he alth contraindications for driving, or contains restrictions in the use of the driving license resulting from the he alth condition, an authorized physician shall, within 14 days from the date of issuing the certificate, provide a copy of it to the competent authority authorized to issue a driving license or a driving license. by tram. Detailed rules for referring to medical examinations, conducting them and issuing medical certificates stating the existence or absence of he alth contraindications for driving motor vehicles are set out in the Regulation on medical examinations of drivers and applicants for driving licenses. On the basis of the examination, an authorized doctor, pursuant to § 10 (1) of the Regulation, issues a medical certificate stating the absence or existence of specific contraindications to driving, which, pursuant to § 10 (4) of the Regulation, should be issued in accordance with the model set out in Annex 5. An authorized doctor issues a medical certificate with a copy, the original of the certificate is received by the examined person, and a copy of the certificate is attached to the medical documentation. In § 12 sec. 1 of the regulation on medical examinations of drivers and persons applying for the authorization to drive vehicles (Journal of Laws of 2013, item 133), the legislator provided for the admissibility of lodging an appeal against the above decision by the examined person or the entity referring the examinations, within 14 dayson receipt of a medical certificate issued by an authorized certificate.
