How do I get a vaccination certificate for COVID-19? What does this certificate look like?

Currently, such a certificate is useful, among others, in people who want to go on holiday abroad - its holders are released from quarantine after returning to the country. How to get them?

Vaccination confirmation is not the same

Everyone who is vaccinated receives a confirmation card at the vaccination point - it shows the name of the vaccinated person, the date of the first dose of the vaccine (along with the batch number of the preparation), the date of the next vaccination and the date of the second dose of the vaccine (also with the serial number).

The card should be kept in your medical records after receiving the second dose. However, it is worth knowing that the card received at the vaccination point is not an official confirmation of being vaccinated against COVID-19. For this reason, it is not recognized abroad, nor are it recognized by e.g. travel agencies.

How to get a vaccination certificate for COVID-19

A certificate of vaccination against COVID-19 can be obtained by logging into the Patient Online Account. The certificate is in the form of a unique QR code, so it cannot be forged.

Already after the first dose of the vaccine, the information about its receipt appears on the Patient Online Account (along with the date, type of preparation, batch number and EAN code). Confirmation of vaccination with the first dose can be downloaded in a pdf file. On the confirmation, apart from the patient's data and PESEL number, there is information about the date of vaccination and which dose of the vaccine was administered.

This is not yet appropriate thoughvaccination certificate. Such a certificate can be generated only after taking the second dose of the vaccine.It is in the form of a digital QR code .

People who do not use the Patient Online Account can obtain such a code directly at the vaccination center after vaccination - it will then be printed directly from the system.

To obtain a COVID-19 vaccination certificate:

  • log in to the Patient Online Account (directly using a trusted profile, ID card with an electronic layer, PKO BP, Inteligo, Pekao SA or Bank Spółdzielczy account)
  • select the "Prevention" tab
  • open the "Vaccinations" tab, where you can find information about the vaccination against COVID-19, vaccination confirmation in pdf and the QR code.
