The decision to transfer a patient to another ward always takes place as a result of consultations between the ward doctors. It is one of the conditions necessary to transport a patient, resulting from the internal regulations of the hospital. The decision-making person must guarantee the continuity of the treatment process for the patient who is to be transferred to another ward. He also has to determine the way of the patient's return, e.g. after an operation from the surgery department to the neurological department. All this requires a decision not only by the head of the neurology department, but also the consent of the head of the other department. A necessary condition for such patient migration is also the patient's informed consent not only to being transferred to another place, to another ward, but also to medical procedures being undertaken by other doctors. Admitting a patient to the ward along with documentation, i.e. the so-called authorization of patient admission, takes into account the date and time of patient transfer. The patient is discharged from the ward he is leaving and enrolled in the ward to which he is transferred. The statistical card on the patient's stay in the first ward is forwarded to the medical office immediately after the patient is handed over. In the case of transferring the patient, it is necessary to issue a referral from the ward from which the patient is transferred to the ward to which he is transferred. In the referral it is necessary to state the reason for the necessity of hospitalization of the patient in the second ward. The ward admitting the patient immediately after admission reports the admission to the admission room of the hospital, fills out a statistical card covering the period of temporary stay in the ward and fills in the history of the patient's stay.
