2. At least one month before the planned departure, you should contact the selected station by phone or e-mail and reserve a place, as well as discuss the details of treatment and set the dates of treatments. A patient going abroad should report to the Provincial Department of the National He alth Fund to obtain the European He alth Insurance Card and the necessary documents that are needed in a foreign dialysis center. Then he should contact the foreign dialysis center (also by phone or e-mail) to arrange all the details. 3. Before departure, medical documentation should be sent to the selected dialysis center, and a copy should be taken with you. People who want to be transported from their holiday destination to the hospitality dialysis center should provide their temporary holiday address just before departure. Transportation to and from dialysis should be provided by the station (covered by your he alth insurance). If the patient goes abroad, he must have his medical records translated into the language spoken by the staff of the station at the stationdialysis. 4. At least one week before the planned arrival, the patient should confirm his presence by phone or e-mail. Detailed lists of Polish facilities that will admit patients for guest dialysis under he alth insurance can be found on the website of the Polish National Association of Dialyzed People - OSOD. There is no shortage of such facilities in seaside resorts. For example, there are 15 of them in the Pomorskie Voivodeship, and 11. in the Zachodniopomorskie Voivodeship. Also, patients who go on holiday to the mountains do not have to worry about their he alth. For example, there are 21 dialysis stations in the Małopolskie voivodship, and 14 in Podkarpacie.
